What if I need to reschedule my appointment?
We ask that you give us at least 24 hours notice if you need to cancel or reschedule your appointment. A $30 fee applies if 24 hours notice is not given.
Why will I being asked to sign a receipt that I have received a copy of the Dental Materials Fact Sheet and a copy of the Office Privacy Practices (HIPPA)?
The Dental Board of California has asked all dental offices to distribute the Dental Materials Fact Sheet to each patient and we are required to collect your signature as documentation of your receipt. The fact sheet is meant to answer questions patients may have about the various types of dental materials we use to restore teeth. We encourage you to read the information and are happy to answer any questions you may have.
All medical and dental offices are required to develop a privacy policy and distribute a copy of the policy to each patient. We are required to collect your signature as documentation of your receipt.
Medical History Updates
In order to treat you effectively, we need to ask you questions about your medical conditions, current medications and supplements you are taking, and any drug allergies you may have. Although you will be asked about changes in your health when you come for your dental appointments, we will also periodically ask you to fill out a new medical history form to help us keep your record current. This information is confidential and will not be shared with anyone other than those involved in your care.
